The District of Parry Sound hosts the program with a post stroke referral process to outside care.

Eligibility

Anyone who has suffered a stroke Caregiver of a stroke patient.

SAFETY: Supports those with limited resources to be taken safely home form the hospital Continuity of Care/Peace of Mind.

Again this is a referral to speak with a program co-ordinator.
